NEW/TRANSFER STUDENT-ATHLETES FORMS only. Please note that these forms are color-coded for my records, so I would rather you got the forms directly from me.  But, if that is not possible please feel free to download the forms here and bring them to me at your scheduled pre-participation screening exam.  PLEASE NOTE: that your personal physician MUST fill out/sign the PERSONAL PHYSICIAN form as per the NCAA.  I cannot accept the generic doctor office form.

 

 

The PARENT LETTER  is for parents/guardians of all student-athletes and should answer many questions you may have regarding the care and treatment of your son/daughter's injuries.

 

 

The following pages include information regarding the NEW NCAA ADHD MEDICATION POLICY, effective August 2009.   Any student-athlete, who takes one of these medications, which is banned by the NCAA MUST download these pages and contact the Athletic Training/Sports Medicine staff immediately so that you understand what must be completed PRIOR to any STAC athletic participation.
